Please follow these steps to add/ report a new incident when
logged in as an admin user.
From
the “Incidents” page create a
new incident by selecting the “New
Incident” button. The Incident
Report form will appear.
Various
information sections will appear depending on the type of incident selected. Certain
fields are mandatory, so the form cannot be progressed or fully submitted unless
information is provided for those fields. Non-mandatory fields are optional.
Complete the relevant details in the sections as they appear (note: the
headings may vary) and hit “Save and Next”.
Incident
Category: Common categories include Environmental, Safety,
Quality, and Security.
Short
description: A brief description should concisely capture the
key details of the incident.
Incident
Type: Depending on the category chosen you will need to select
from a range of incident types, e.g. First Aid, Near Miss or Hazard.
Incident
supporting photos: Upload any relevant photos, videos, or
documents.
Person
Reporting the Incident: Defaults to the logged-in user.
Location
Details: Date, time, and place of the incident.

Involved in the Incident
Witness(es)
to the Incident
Injured
personnel (if applicable): Note the person who was injured (must correspond
to one of the Persons Involved), and the description of the injury.
Injury
Report (if applicable): Includes sections on “Mechanism of
injury”, “Nature of injury”, “Body location”, and “First
Aid”.
Alcohol
or Drugs (leave blank unless testing was performed).
Immediate
Corrective Actions: Record any immediate actions that were taken
at the time of the incident.
Incident
Reported To: The person notified about the incident and the time
of notification.
Incident
Severity: Provide a rating of the likelihood and consequence of
the incident occurring. Select the radio buttons or click on a coloured risk level rating cell.
Distribution
List: Populates with a list of people who will receive a
notification of the incident once submitted.
After completing all required fields, select “Submit” located at the bottom right of
the “Distribution List” section.
A confirmation of the successfully submitted incident will
appear. Select “Return to Incidents”
to navigate back to the “Incidents”
page. The new incident will show in the
“Awaiting Assignment” status. Click on the “three dots” to “Print/
View” or “Edit” the report.
