How to add an incident

Please follow these steps to add/ report a new incident when logged in as an admin user.

  1. From the “Incidents” page create a new incident by selecting the “New Incident” button. The Incident Report form will appear.
  2. Various information sections will appear depending on the type of incident selected. Complete the relevant details in the sections as they appear (note: the sections may vary) and hit "Save and next". Certain fields are mandatory, so the page cannot be saved or progressed unless information is provided for those fields, which will be highlighted in red. 
      • Incident category: Common categories include Environmental, Safety, Quality, and Security.  
      • Incident title: A brief description should concisely capture the key details of the incident.
      • Primary incident type: Depending on the category chosen you will need to select from a range of incident types, e.g. First Aid, Near Miss or Hazard.
      • Incident details: Enter the relevant information to describe the incident in a factual manner.
      • Incident supporting photos: Upload any relevant photos, videos, or documents.
      • Person Reporting the Incident: Defaults to the logged-in user. This can be changed if the admin is lodging an incident on behalf of someone else.
      • Location details: Record the date, time, and place of the incident. 
      • Person Involved: Record the people involved in the Incident. Multiple people can be added.
      • Witnesses to the Incident
      • Injured personnel (if applicable): Note the person who was injured (must correspond to one of the persons involved), and the description of the injury.
      • Injury Report: (if applicable): Includes sections on “Mechanism of injury”, “Nature of injury”, “Body location”, and “First Aid”.
      • Alcohol and drugs (leave as "No" unless testing was performed): 
      • Immediate corrective actions: Record any immediate actions that were taken at the time of the incident.
      • Incident Reported To: The person notified about the incident and the time of notification.  
      • Incident severity: Provide a rating of the likelihood and consequence of the incident occurring. Select the radio buttons or click on a coloured risk level rating cell.
      • Who have you already informed?: The person the incident was reported to and the time of notification.  
      • Distribution List: Populates with a list of people who will receive a notification of the incident once submitted.
  3. After completing all required fields, select “Save and Next” located at the bottom right of the “Notifications and distribution” section. 
  4. The Review section, allows you to check the information before you "Submit" the incident.
  5. A confirmation of the successfully submitted incident will appear with the new incident number. Select “Return to Incidents” to navigate back to the “Incidents” page.  The new incident will show in the “Awaiting Assignment” status. Click on the “three dots” to “View / Print” or “Edit” the report.
