Understanding each "Form Field" answer option in an audit form template

Background
When building an audit form template using the Form Builder, “Form Field” is used to set up questions and responses. The "Form Field" section has various answer options.

This article provides an explanation of each “form field answer option” within Form Builder.

Overview of "Form Field" answer options
Text Box
Allows users to input a single line of text, ideal for brief answers or concise information. Field Type can be set as Alphanumeric, Date/Time etc. Prefixes or Suffixes can be added e.g. Hours, Kilometres.

Text Area
Allows users to input multiple lines of text, used for questions requiring detailed responses or extended explanations. A maximum word length can be set if required to limit responses.

Multiple Choice
 Lets users select an answer from a list of options - this is useful for Yes/No questions. “Only Selected Option” allows for only one correct answer (tick the radio button to indicate the compliant response). Responses can be overtypes, added or removed. The answers can be displayed as radio buttons or as a dropdown list.

Checkboxes
Provide a predefined list of options that users can select or deselect, which is beneficial for receiving multiple responses from a set of options. 

File Upload
This lets users upload photos or files as part of their response. Select “Multiple File Upload” to allow more than one file/photo to be uploaded.

Digital Signature
Requires users to input their signature digitally, great for acknowledgements or consents.
