How to report an incident

Learn how to report a safety incident using Rapid Worker.


When to Report an Incident

Report any safety-related event, including:
- Injuries - Any workplace injury
- Near misses - Events that could have caused injury
- Property damage - Damage to equipment or facilities
- Environmental incidents - Spills, releases, etc.
- Safety violations - Observed unsafe behavior
- Equipment failures - Malfunctioning equipment

Steps

Step 1: Access Incident Report

From the Dashboard, tap Report an Incident.

Step 2: Select Company (if applicable)

If you have multiple packages, select which company to file the report with.

Step 3: Add Photo Evidence (Recommended)

Tap Add Photo to include photos of:
- The incident scene
- Any damage
- Relevant conditions

You can use Camera or Gallery.

Step 4: Enter Incident Details

Fill in the incident information:
- Title - Brief description (3-200 characters)
- Description - Detailed account of what happened (10-2000 characters)

Step 5: Add Date, Time, and Location

Enter when and where the incident occurred:
- Select the date
- Select the time
- Choose the location from the list

Step 6: Select Follow-up Contact

Choose who should be contacted for follow-up:
- Contact me - You'll be contacted
- Contact someone else - Enter their details
- Anonymous report - No contact information provided

Step 7: Review and Submit

A confirmation screen shows your report summary. Tap Submit Report.

Step 8: Receive Confirmation

You'll receive a confirmation with an incident reference number. Save this for your records.

Offline Incident Reports

You can start an incident report even without internet connection:

  1. Start your report as normal - the app works offline
  2. Fill in all details - incident information is saved locally
  3. Add photos - captured images are stored on your device
  4. Save offline if you can't submit immediately
  5. Auto-sync - when you reconnect to the internet, your report is automatically submitted

Important: Report incidents as soon as possible, even if you're offline. The offline feature ensures your report isn't lost.
