How to edit or re-open a completed investigation

Once an investigation has been completed and submitted, there may be situations where it needs to be subsequently amended. The process for editing or re-opening an investigation varies slightly depending on the workflow status of the incident.

Awaiting approval: In this stage the Investigation has been completed but not yet “Approved.” To open the investigation, click on the “menu dots” and select the “Edit investigation” option. Minor changes will be saved. For major changes it is recommended to select the “Reject” option to move the incident back to “Investigating” status.

Corrective Actions / Awaiting to be closed / Closed: In these incident statuses, the corrective actions have been approved. The actions may be open or closed. To return the incident back to the “Investigating” status, click on the “menu dots” and select the “Re-open investigation” option. Any open or closed corrective actions resulting from the original investigation will be “recalled”. The investigation will need to be progressed as normal to reinstate the corrective actions.

Note: when an incident status is changed back to "Investigating," notifications will issue.
