How to approve or reject an investigation
After submitting the investigation, investigators will be
notified. The incident will now be in “Awaiting approval” status. A
responsible administrator needs to review the investigation findings, root
causes and corrective actions, then approve (or reject) the investigation. Once
the Investigation has been approved the people assigned Corrective Actions will
be notified.
To approve or reject an investigation, please use these steps:
- Find the incident currently “Awaiting approval” using
the “Keyword” search filter on the left side of the “Incidents”
page, type in the incident number or keyword. Alternatively, click on
the “Awaiting approval” status square.
- Click on the “menu dots” at the right of the incident

- Click on “Approve/reject.”
- To approve the investigation, select the “Approve investigation”
radio button and press the “Approve investigation” button ant the bottom
of the window. This will trigger the notifications to the people who have been
assigned a Corrective Action.
- To reject the investigation, select the “Reject investigation”
radio button, add your comments in the “Rejection reason” text box detailing
why you have rejected the investigation. To submit, select the “Reject investigation”
button at the bottom right. The comments will be sent to the investigation team
and the incident will revert back to “Investigating” status.
Tip: If corrective actions are not visible in the
system following the completion of an investigation, it indicates the
investigation is still awaiting approval. Approving the investigation, also
approves the corrective actions.
