How to add Scoring and Weighting in audit forms

Scoring” and “Weighting” can be added to Audit Form templates for all “Form Field” answer types. This allows a percentage score to be shown to indicate the level of compliance for a completed audit. For example, a score 90%, means 9 out of 10 responses were correct.

Scoring” applies a value to the answers. “Weighting”, if greater than 1, will apply a multiplier effect to the particular question (e.g. a particular question may have a greater weighting than other questions).

To set up scoring and weighting in an audit form template, please follow the instructions outlined below.

Scoring and weighting an audit form template

  1. Ensure the the Audit Form template has “Form Scoring” activated.

  2. In the “Form Field” window, activate the “Weight” and “Scoring” switches.

  3. Set the “Max Weight”, the standard default is “1”.

  4. For “Multiple Choice” or “Checkboxes” set the score value(s). Usually the “Compliant” answer is set to “1”, scoring “1/1”.

  5. If “Non-Compliant” answers are left as “0”, they will score “0/1”.

  6. Scoring for “NA” (Not Applicable) answers can be left blank as they will automatically score “0/0”.

  7. Click the “Save” button.

 

The weight & scoring functions can be used in the following ways:

Weight ON, Scoring ON = Predetermined score for the chosen answer will be multiplied by the weighting.
Weight ON, Scoring OFF = User can select a score up to the maximum value of the weighting (no predetermined score).
Weight OFF, Scoring OFF = No weight or score for the question.
