This next step of the Permit to Work Info Pack provides an overview of Form Builder and its integral role in the Permit to Work system.

Form Builder is the internal Workforce platform used to create and customise forms and permits. For the PTW module, it allows you to build and edit permit forms that contractors can fill out and submit digitally.
Because different clients handle Permit to Work uniquely, Form Builder offers unlimited flexibility to create any permit type required. Typically, clients use this tool to transition their existing paper permits into a streamlined, digital format on our system.
How to create a new permit
The PTW forms are managed directly within the Permit module. To add a new form:
- Go to Permit → Settings
- Enter the Manage Permit Forms area
- Press Add New Form:

*For existing permits, you can simply press the Form Builder option to edit the form

Adding fields within a permit
Within the permit’s Form Builder, you can add information / questions using the Add New Field option.
To add a field:
- Press the Add New Field option
- Select the relevant field type
- Enter any information if required
- Press the Save button:

Understanding the Field Type options
The list of options you have to add are the following:

Creating questions
Questions within Form Builder are set using the Form Field option within the editor. To add a question:
- Press the Add New Field option
- Select the Form Field option
- Enter your question
- Select your answer type
- Press the Save button

Understanding the Answer Type options
The list of Answer Types you have to add are the following:

Conditional Formatting allows you to dynamically show or hide fields based on a user's specific answers, keeping the form clean and relevant. You can trigger these visibility rules using the following conditions:
- Equals / Does Not Equal: Triggers when a specific answer is (or isn't) selected.
- Is Empty / Is Filled: Triggers based on whether a field has been left blank or completed.
- Press the Conditions option
- Press Add New Condition
- Select the question and the response you require to trigger the condition
- Select the following fields you would like impacted by the conditioning
- Press the Save button:

Exit questions
When creating a permit, the questions you configure in Form Builder are explicitly displayed to the worker while they are submitting the permit.
However, it is common for clients to request exit questions for work orders, such as asking the worker whether the work order was fully completed upon departure. These exit questions are managed in a completely different area of the system.
